General Forms

Listed below are general forms used by the customers of South Central Indiana REMC :

• Installation Agreement - this form must be completed and signed before any new construcution work is performed on a customer's property.

• Pole Release Form - this form must be signed before a customer may take a used SCI REMC pole for personal use.
